    Symptoms of Complex Kidney Stones

Large kidney stones do not always cause immediate or obvious pain. Some cause persistent discomfort that comes and goes. Others are discovered on imaging before symptoms ever become severe.

See a urologist if you are experiencing any of the following:

Persistent or recurring pain in the side or back, below the ribs

Step 1 — Symptom and Medical History Review

Dr Azhari will listen to your symptoms and go through your full medical history, including any previous kidney stone episodes, infections, or changes in kidney function.

    road map icon

    Step 2 — Physical Examination

A focused physical examination will be conducted to assess areas of discomfort and your overall condition.

    road map icon

    Step 3 — Diagnostic Imaging

Depending on your presentation, imaging such as a CT scan, ultrasound, or X-ray will be arranged. CT imaging is particularly important for large or complex stones as it reveals stone size, number, location, density, and the kidney’s anatomy, all of which inform treatment planning.

    Step 4 — Treatment Discussion

Dr Azhari will explain what the findings mean and walk you through the treatment options appropriate for your specific case. For large or complex stones, this may include a discussion of minimally invasive surgical options such as PCNL.

    Step 5 — Prevention and Follow-Up Plan

You will leave with a clear, personalised plan, including follow-up imaging schedules, kidney function monitoring, and lifestyle advice aimed at reducing the risk of stones recurring.

Percutaneous Nephrolithotomy (PCNL) for Large Kidney Stones

PCNL is a minimally invasive surgical procedure for large or complex kidney stones that are not suitable for shockwave therapy or scope-based approaches.

Dr Azhari will assess your individual case to determine whether PCNL is the appropriate treatment for your condition.

    The Procedure

    The Procedure

A small incision is made in the back to create a direct access channel to the kidney. The stone is then fragmented and removed through this channel.

    • Performed under general anaesthesia
    • Hospital stay is typically 1 to 2 days
    • Most patients return to normal activities within a few weeks
Stones PCNL May Be Used to Treat

Stones PCNL May Be Used to Treat

    • Large single stones exceeding 2 cm
    • Staghorn calculi — branched stones filling multiple parts of the kidney
    • Multiple stones within the kidney
    • Stones in locations difficult to reach through other approaches
    • Stones that have not responded to previous treatments such as ESWL

    Variations of PCNL

    Standard PCNL Larger access channel
    Mini-PCNL Reduced access size
    Ultra-Mini PCNL Further reduced access
    Micro-PCNL Smallest access available

Dr. Azhari will determine the most appropriate technique based on your imaging and clinical assessment.

    Potential Risks

As with any surgical procedure, PCNL carries potential risks.

    • Common minor effects — temporary blood in urine, mild fever, discomfort at the puncture site — typically resolve within days
    • Serious complications are uncommon
    • All relevant risks will be discussed with you before any procedure is scheduled

All treatment recommendations are based on individual clinical assessment. Results may vary depending on each patient’s condition.

Same-day appointments available. All enquiries handled with strict confidentiality.

    Frequently Asked Questions

How long does PCNL surgery typically take?

PCNL surgery duration varies based on stone size, number, and complexity. Simple cases with single large stones may take approximately 60-90 minutes, while complex staghorn calculi or multiple stones can require 2-3 hours.

Is PCNL painful compared to other kidney stone treatments?

PCNL is performed under general anaesthesia, so you won’t feel anything during the procedure. Post-operative pain is typically mild to moderate and well-controlled with medications. Pain levels vary among patients in the first few days, improving thereafter. Compared to open surgery, PCNL causes less pain due to the small incision.

What is the success rate of PCNL for large kidney stones?

PCNL can achieve stone clearance for large kidney stones in a single procedure. Success depends on factors including stone size, location, and composition, as well as kidney anatomy and surgeon experience. Modern techniques like flexible nephroscopy improve access to difficult areas. If residual fragments remain, they may pass naturally or require additional treatment.

Can PCNL be performed on both kidneys simultaneously?

Bilateral PCNL (treating both kidneys) can be performed as staged procedures or simultaneously in selected cases. Single-session bilateral PCNL may be considered for younger, healthy patients with good kidney function and bilateral large stones.

How soon can I return to work after PCNL?

Return to work timing depends on your job’s physical demands and recovery progress. Desk-based work may be possible within 7-10 days, while physically demanding jobs may require 2-3 weeks off. Light activities can typically be resumed within one week, but heavy lifting should be avoided for at least two weeks.

    Take the Next Step

Large or complex kidney stones benefit from early assessment. An evaluation will help clarify your diagnosis, your treatment options, and what can be done to protect your kidney function and prevent recurrence.
